The National Highway Traffic Safety Administration say it has launched an investigation into why General Motors Co. waited until this month to recall an ignition switch flaw it discovered a decade ago.

The agency could levy a fine of as much as $35 million against GM if it concludes the company failed to act quickly to fix a recognized defect.

GM describes itself as "deeply sorry" about the problem, which has been linked to 31 crashes and 13 fatalities. Earlier this week it expanded to 1.6 million a recall to replace ignition switches that could be jostled into the "off" position by a bump in the road or the weight of an overladen key ring.
